Birth Registration

Every baby born in Ontario must be registered. Registration provides official documentation of the birth to the government and allows you to apply for documents (including a birth certificate) for your baby. There is no cost to registering your baby. Birth registration in Ontario is a two-step process:

  • Statement of Live Birth Form, is completed by parents of newborns and submitted to ServiceOntario
  • Notice of Live Birth Form, is completed by qualified medical staff and submitted to ServiceOntario

Service Ontario now offers a "five-in-one newborn bundle": using the 5-in-1 newborn bundle is free and it is quicker to register a birth online than to submit a paper Statement of Live Birth form by mail.

If you register the birth online, you can also do the following at the same time:

  • Get a birth certificate or birth certificate with parental information
  • Apply for a Social Insurance Number (SIN)
  • Sign up for Canada child benefits (this includes the Ontario specific child benefits)
  • Request an Education Savings Referral
